Residency Affidavit (RA) Renewal for 2019-2020

RESIDENCY IS DEFINED AS LIVING IN THE HOME EXCLUSIVELY.

All RA's must be renewed annually.
If you do not renew your student's RA, they will not be allowed to continue enrollment with Yukon Public Schools.

You now have three options to renew your RA. Enclosed you will find a RA form. Please complete this form and note that the RA form must be notarized. DO NOT SIGN RA form unless in front of a notary. **Notaries will be on site during dates and times listed below.

  • Option #1: Evening session will be held on June 4th, 5th, and 6th from 4:00PM - 7:00PM in the enrollment center, located in the 6th grade building. The person, whom you are living with, must accompany you to sign the affidavit.

  • Option #2: June 3rd, 4th, 5th, and 6th you may come during the hours of 7:00AM - 3:00PM, closed 11:30 – 12:30. The person, whom you are living with, must accompany you to sign the affidavit.

  • Option #3: (QUICK EXPRESS) Parent, guardian, or designated adult may bring the completed and notarized form, along with copies of required documents during any of the dates and times listed above. (See requirements below) Copies will not be made.

REQUIRED PROOF OF RESIDENCY

  1. Valid ID
  2. Two Current (within 30 days) utility bills (ex: gas, water or electric)
  3. Mortgage, deed, lease, or rental agreement

    If you are residing with a person who lives at one of the mobile communities or apartments, they will also need a letter from the manager stating that they are aware of your residency at that address.

Important Information

  • UTILITY BILL: RA's will be required to provide a current (30 days) utility bill in the name of the primary resident.

  • ADDRESS CHANGE: If you have established your own residence, in YPS district, you will need to provide the required proof of residency documents to your home school, upon receiving this letter.

  • MOVED OUT OF DISTRICT: If you have moved out of the YPS district, you must notify your school site. To apply for a transfer for the next school year, visit www.yukonps.com, under the enrollment tab.

    If you are in need of assistance, please contact the Yukon Schools Enrollment Center at 405-265-1300. Students are not required to
    attend.
     

Oklahoma Law Book Section 14. Residence of Child - Attendance in Transportation Area

When used in this section, the residence of any child for school purposes shall be: The school district in which the parents, guardian, or person
having legal custody holds legal residence. Custody (as used in A.1) means legal custody; physical custody alone is not sufficient to change a child’s
residency for purposes of admission to school district.

The following students shall be considered legal residents of the District: Students whose parent, legal guardian or legal custodian holds legal
residence in the District. The Canadian County Sheriff’s Office would like to advise all parents, guardians, and Yukon School District residents of the Oklahoma State Statute on determining a child’s residence. Charges will be filed with the Canadian County District Attorney’s Office if it is found that any party completing any residency paperwork, or a residency affidavit, for the Yukon Public School District has falsified any of the required paperwork. Please read the statute below:

Title 70 O.S. § 1-113 A. 1. Determination of Child’s Residence (OSCN 2015)

A. When used in this section, the residence of any child for school purposes shall be:1. The school district in which the parents, guardian, or person having legal custody holds legal residence.

B. Each school district board of education shall adopt a policy establishing the requirements for student residency for that district which provides
for residence as described in this paragraph. Within the discretion of each school district's board of education, the policy may but is not required to allow for establishment of residency by affidavit when an adult, whether a relative or not, who does not fall within one of the categories listed
above, who holds legal residence in the school district, and who has assumed permanent care and custody of the child files an affidavit with the school district attesting that they have assumed custody and the reasons for assuming custody. Any policy allowing the establishment of residency by affidavit shall require the adult who provides the affidavit to affirm in such affidavit that the custody arrangement is permanent and that the adult contributes the major degree of support to the child. If the school district policy allows establishment of residency by affidavit, any person who willfully makes a statement in the affidavit which the person knows to be false shall, upon conviction, be guilty of a misdemeanor punishable by imprisonment in the county jail for not more than one (1) year or a fine of not more than Five Hundred Dollars ($500.00) or both such fine and imprisonment. Each school district shall include in its policy on residency any documentation necessary for the administration of the policy.
